About The RoleLocation: Community based in Grimsby and North East Lincolnshire, with some home working. Contract: Fixed term contract until March 2024, with possibility of a permanent position.  We reserve the right to close this vacancy early if we receive sufficient applications for the role. Therefore, if you are interested, please submit your application as early as possible.We are hoping to welcome an approachable and knowledgeable Dementia Adviser to work across the North-East Lincolnshire area. This role offers a rewarding opportunity to provide support and guidance to people affected by dementia; helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives. Within the role you would also provide support to people who have concerns about their memory or are within the diagnostic pathway.This role supports people before and after their diagnosis; in the community or in their homes, where we interact compassionately face to face, by telephone and on-line to give people the tools and knowledge to make informed decisions about their future. In this role, you will also assist people who use our service to access other services, by signposting and referring, as well as support the facilitation of monthly Dementia Café’s. You will provide community based, face to face services for an increased level of support for more complex cases. Managing referrals, assisting clients in assessing their information and support needs in a person-centred manner is also key to this roleAbout you

  • Working experience of assessing people face to face with an ability to assess their needs.
  • Deliver advice and guidance with a non-judgmental approach.
  • Able to travel independently around North-East Lincolnshire.
  • An understanding of dementia and the needs of those living with dementia and their carers would be an advantage but, if you have had experience supporting with other challenges this would be taken into consideration.
  • Experience of leading groups and able to demonstrate an understanding of the value of group activities to people affected by dementia.
  • It is desirable that you have experience of coordinating volunteers and liaising with statutory and voluntary agencies.
  • Have a commitment to your continued personal development and learning.
